Pre-registration Info You can pre-register by printing out the entry form and waiver and faxing them to (352) 846-1079. See below for the entry form and appropriate waiver. Tournament Info One the day of the tournament, registration for Juniors will be from 8am to 10:30am. Seniors and Masters, till 12:00pm. We will accept personal checks and cash. Scroll down to see a map of the tournament location. Entry fees during pre-registration $35 for one division $60 for two $80 for three Entry fees on the day of the tournament $45 for one $75 for two $100 for three There will NOT be any food or drinks available to buy at the tournament. Unfortunately, University of Florida policy prohibits clubs from selling food on campus. There are many shopping centers and places to eat within a few miles of the tournament. Also, food and drinks other than water are not allowed in the gym where the tournament will be held. Please eat outside. Thank you in advance for your cooperation. Forms Here is the entry form for all competitors. Entry Form There is a waiver that needs to be filled out to compete at the tournament. If a minor is competing at the tournament without a parent present, you can download the waiver below and sign it, then bring it to the tournament. (Please disregard the swim test section of the waiver and the part that says to fax it in.) For competitors UNDER THE AGE OF 18, use the following waiver: Minors' waiver (as Word document).
